Description | The 'Zorgpad werk' care path offers first-line health workers and service providers on the labor market guidelines on timely and optimal coordination of therapeutic treatments at work (resumption). The methodology of care pathways is recognized by RIZIV as good practice. We develop a transversal care path independent of specific diagnoses, but facilitate the focus on work so that the chances of job retention or fast job (re) integration are great. To achieve this, we vote the ICF data set for labor reintegration on the ICF checklist for medical treatment and provide the service providers with a methodology that allows customers / patients to motivate their own process. This innovative service can generate very important financial and social payback effects. In this project primary we support care providers (such as the general practitioner, the kiné and the psychologist) to provide good advice for action to their clients / patients in time (again) to orientate to work. These action recommendations are unequivocal, scientifically substantiated and geared to the concrete support offer (work-care) in the region. By using WRAP (Work / Wellness Recovery Action Plan), the customer comes at the wheel of his steps to work. |
